Below is the format you need to fill out fully in order to get your password reset.
Fresh Hotel username:
Email address:
Country of residence:
Approximate registration date:
Last sign in date:
Internet Protocol (IP) address:
Internet service provider (ISP):
Username of other account(s) on the same IP Address (optional):

* Note: You are limited to 3 password resets a month due to attempts of account hijacking. *
* Note: All information is required or you will not be granted a password reset. *
