Include username of the staff that added the ban : The moderator's name was Ryan

Reason for the ban : Didn't specify a reason

Why the ban should be lifted? : This was a while ago, and I have been nicer and don't act like this anymore. If my IP ban is lifted, I will not act like this anymore.
